SAMPLE Internship Proposal: This document is used ONLY after receiving an internship. This sample may be used as a guide to develop your own proposal for academic credit. When finished, it should be submitted to comm-internships@illinois.edu.
Employer Evaluation Form: At the end of your internship this form will be used by you to evaluate your internship/work experience. This form will need to be submitted with your internship materials .
Intern Evaluation Form: At the end of your internship this form will be used by your on-site supervisor to evaluate your performance in the internship. You will need to provide this form to your employer. It must be completed and submitted with your internship materials in order to receive a grade.
